Brief description of service

Shropshire Enablement Team (SET) provides a specialist health rehabilitation service in the community for people of working age who have been disabled by accident, or illness.  There are four main diagnoses seen by the team:

  • Head injury due to trauma
  • Brain injury due to stroke, encephalitis, or tumour
  • Progressive neurological conditions, such as MS, MND or Parkinson’s Disease
  • Chronic Fatigue Syndrome/ME

 

 

Where is the service provided?

The service is provided in a variety of settings: people’s homes, clinics, hospitals, client workplaces, colleges and day centres.

 

When is the service provided?

The service aims to meets people’s differing needs at the distinct stages in their illness/ disability.  For example, after an acute episode such as a stroke, it is often necessary to get involved soon after discharge from hospital. With longer term conditions, we see people as their needs change, e.g. returning to learning or employment, moving home etc.

How to access the service

For most diagnoses, we have an open referral process, whereby people can refer themselves or family members. We also get referrals from GPs, Consultants, therapists and Nursing colleagues.

 

For people with Chronic Fatigue Syndrome, we do require a referral from a medical practitioner.
